11/18/2023 EnchantaVaughn Approved 1.Behavior: QMHP-C greeted Ashiyah upon arrival to the home setting. Upon arrival to the home setting, Ashiyah appeared to be in a positive mood. QMHP-C informed Ashiyah that today’s session wouldn’t be about anything particular just allowing her to verbalize her thoughts, feelings, and emotions with QMHP-C. Ashiyah reported she really did not have much to talk about just continuing to work on bettering herself. Mother reports these behaviors last abou 30 minutes in duration. 2. Behavior: Ashiyah participated in the session and did not need any redirection from QMHP-C during the session. Ashiyah reported she tries to be better than she is from the day before so she will continue to take progress towards her goals. Ashiyah reports when she does not feel like talking to anyone she will talk to anyone with behaviors lasting 3-5 hours. 1. Intervention: QMHP-C allowed Ashiyah to participate in free association talk time. Ashiyah wanted to talk about male/female relationships. QMHP-C talked with Ashiyah about the characteristics of one of her male peers. QMHP-C inquired as to whether the peer has positive or negative quality characteristics for increasing positive peer relationships. QMHP-C assisted Ashiyah with processing her feelings and emotions. QMHP-C provided Ashiyah with role playing activities. QMHP-C conducted a review of the previous session. 2. Intervention: QMHP-C asked Ashiyah relevant questions to obtain Ashiyah’s level of comprehension. QMHP-C intervened by allowing Ashiyah to talk about her feelings as it related to the previous session about her father and how he gets on her nerves and how she feels as though she is not treated like a child of his. Ashiyah was offered praises by the QMHP-C for the positive contribution made by Ashiyah to remain positive during the session. 1. Response: Ashiyah maintained a jovial and talkative attitude during the individual counseling session. Ashiyah mentioned negative comments about her peers and was redirected by QMHP-C several times. Ashiyah readily admitted this peer may not be the best peer and that she has been wanting to fight this peer for a long time to the point that she was going to meet her at her house on Ruffin Road and fight her because she doesn’t like her over a male that she likes and another peer is talking about it. Ashiyah indicated to QMHP-C that she really likes this male and that this peer is going to have to back up. 2. Response: Ashiyah was respectful while QMHP-C was talking about the qualities of relationships. Ashiyah made positive and negative comments about her male/female peers. Ashiyah stated that she has more positive qualities than negative qualities. QMHP-C Interpretation of client’s progress towards today’s objectives: Ashiyah continues to struggle with being able to resolve conflict within the home, school, and community. QMHP-C Plan of Action: VACCD staff will continue to use role-playing, modeling, and behavior rehearsal to teach Ashiyah to become open and responsive to praise and encouragement.
